Outstanding Service Award
For Outstanding Contributions to Animals or the WSU College of
Veterinary Medicine
Nomination Deadline: February 1, 2010
Eligibility
The Outstanding Service Award is granted as a testimonial to an individual
who has made outstanding contributions to animals and/or the WSU College of
Veterinary Medicine. All nominations for this award should contain specific
reference to the nominee's activities related to that service.
The Award
The awards consist of a plaque that will be
presented during the College of Veterinary Medicine Celebration of
Excellence awards banquet. In addition, each recipient will have his/her
name added to a plaque in Bustad Hall.
Nomination Process
- Please complete the nomination form below.
- Arrange for two additional recommendation letters to be sent from
others in support of this nomination.
- Additional sheets and supportive documents can also be attached.
(The more information you can provide, the better the chances are for
your candidate.)
Nomination form can be completed online and then printed, or the
form may be printed blank and completed by hand.

11. Name five (5) outstanding contributions to animals and/or the WSU
College of Veterinary Medicine.
For example: Awards received; special recognition by peer groups
(professional or non-professional); demonstration of commitment to animals
or to the college.
