DC for Clients and Facilitators

DC Client Evaluation Instructions

 Evaluations

Completed evaluations need to be returned to your case facilitator within 7 days of the end of the DC exercise.  Your facilitator will return the completed forms to your DC clinics and individual students as part of his or her facilitator evaluations.

If necessary, evaluations can be returned by FAX, mail, or FedEx - just email or call your facilitator for a FAX number (509) 335-8529 or shipping address, and a pre-paid FedEx account number. There should be one set of forms for each of your DC clinics.

One "Client's CLINIC Evaluation" per clinic  (REQUIRED)

  • Use this form to discuss how the group worked together as a whole and how they worked together to present the case to you.  There are further instructions on the evaluation form.
    • There are TWO EASY OPTIONS for completing the clinic evaluation.  See download links at bottom of this page.
      1. 2 hard (paper) copies of the clinic evaluation form were provided in your DC Client Handbook.  You can just fill out one eval for each of your clinics and pass the completed forms to your facilitator.  If you need another form, just use the link at the bottom of this page to download and print as many as you like.
      2. Alternatively, you can download an electronic copy of the form, complete it online, save the form to your computer, and then just email it as attachment to your facilitator.

Example of a Clinic Evaluation

One "Client's Individual Student Evaluation" per student  (OPTIONAL)

  •  It's important that students receive feedback - both positive input on their perceived strengths and constructive input regarding areas that you believe need improvement.

  •  As a client, you may choose to provide only oral feedback (in the de-briefing sessions on Wednesday afternoon)  OR  oral and written feedback - using the provided form for the latter.  Helpful hints on providing feedback are included at the back of the DC Client Handbook (and through the Feedback Resources links a bit further down this page).

  • It's important that students see your input as constructive and not destructive.  Please be sensitive and seek help from your facilitator or other members of the DC Group if you have questions or concerns.

  • You may use the forms as you deem best and should not feel obligated to fill out a form on every student.  However, please note that our students ARE typically hungry for your input - especially when that input is specific.

  • On the forms, please reserve the "Exceeds Expectations" rating for clinics or students that are TRULY exceptional.

  •  If you have the time and interest, you may also choose to make separate appointments with your students on Thursday AM in order to provide more individualized feedback face-to-face.  Your facilitator can help organize this.

  • Feel free to type or print your comments and attach those comments to the appropriate page.  Do not attach your Daily Notes.  Click here for an example

2011: We've revised the evaluation forms so that you can quickly fill them out online and email to your facilitator.  Let us know what you think!  Our hope is that this will be easier for you!
