Pre-DC

Facilitator "To Do" Checklists   - Please check these out and plan ahead.  Try to make sure your client feels comfortable & prepared. 

  • Pre-DC   (prior to the DC - starting several weeks before the exercise)

Prior to Monday, you must have prepared [1] Clinic Scenario  (need to revise for email delivery) and [2] Client's Day 1 script.

[1]  Clinic Scenario: Geographic location, Clinic capabilities, and Appointment (signalment & client complaint).  Give the clinic capabilities some careful thought so that it fits well with your case.  [2]  Client Script:  Remember that clients should receive information for Day 1 ONLY, so that they don't know what's coming and don't know the Dx.  Additional information is provided during each submission period & the facilitator should be working closely with his/her client.

  • DC Week   (a checklist of things to do and remember during the exercise)

  • After the DC   (the evaluation process, final disposition of your case record, return of notebooks, etc.)

Day 1

Agenda/Script for First Meeting  (A potential itinerary for your first Monday AM meeting with students.  You can fill in the room numbers & give this to them if you want.)

Day 2-3, including De-Briefing

De-briefing Meeting - agenda/script    (a proposed itinerary you can use for your debriefing sessions)

Forms - to insert into medical record daily   (These are now included in the students' Clinic Folders, so you shouldn't need to insert them.  They are intended to help the students keep their record organized and get their final tasks completed) 

 

DC Grand Rounds

Facilitator's evaluation of DC Grand Rounds presentations   Hard copies also provided in rounds. Attach one to your clinic evaluation.

EVALUATION

#1: Post DC - Final tasks and facilitator's evaluation form       NEW FOR 2011 - Revised for online completion.  Follow links to form.  Save a copy and email.

#2: FORM - Rubric for providing feedback on SOAP's and MPL   (You can use this simple rubric to help assess academic SOAPs.  Attach to your clinic evaluation.)

 

#3: FORM - Facilitator's evaluation of DC Grand Rounds presentations  See above under DC Grand Rounds - attach one to each clinic evaluation.

#4: FORM - Client's evaluation of each clinic  Attach your client's evaluation to yours.  LINK to form on DC Client Page

 

FORM - Students' Evaluation of DC2    (and their classmates presentations)

  • Typically provided as hard copies at DC Grand Rounds, students hand these in after the exercise
  • This is also the form they use to score their peers for the 5 peer points
  • If you want, you can hand these out (1 per student) at the Debriefing session

CLIENT INFORMATION  

The facilitator provides case-specific information - emailed to client & hard copies inserted into Client Handbook.
